Cost of Membership is $345.00 for the initial member. $300.00 for each additional family member and $150 for children under 18 years old.
Membership Services Include:
- 20% off on all our regular dental fees
- Two (2) Comprehensive Examinations by the doctor in a 12 month period
- Two (2) Standard teeth cleanings in a 12 month period
- All necessary X-rays in a 12 months period (includes full mouth) if needed
- Oral Hygiene Instructions
- Oral Cancer Screening
- Periodontal Screening
- One (1) Emergency Exam included
- Eligible family members included are a spouse and dependent children
- D.H. Family Dentistry Savings Savings Club is renewable on a yearly basis.
- Membership is non-refundable and all services are only usable during the 12 month period after the membership is paid in full.
